Hospice Assistant Director of Nursing (ADON)

Location: San Jose, CA

Schedule: Full-Time

Setting: Hospice Care

About the Role

Join a compassionate, mission-driven hospice team dedicated to providing exceptional end-of-life care. We’re seeking a skilled and empathetic Assistant Director of Nursing (ADON) to help lead clinical operations, support staff, and uphold the highest quality standards for patients and families.

Responsibilities

Support the Director of Nursing in managing day-to-day clinical operations
Provide mentorship and guidance to field and office nursing staff
Oversee patient assessments, care coordination, and symptom management
Ensure compliance with hospice policies and state/federal regulations
Participate in interdisciplinary case conferences and performance improvement
Foster teamwork, collaboration, and clear communication across departments

Qualifications

Active RN license (BSN preferred)
2+ years of hospice or home health experience (leadership preferred)
Knowledge of hospice regulations and compliance requirements
Excellent communication, problem-solving, and leadership skills
Proficiency in EMR/documentation systems
