Compensation: $105,000 to $120,000

Position Summary:
We are looking for a Physician Assistant (PA-C) who enjoys working independently while being backed by a supportive clinical team. This is a mission-driven, patient-first opportunity to truly connect with your patients and influence their health outcomes.

Responsibilities:

• Conduct primary care visits in assisted living and memory care settings
  
• Develop care plans, manage medications, and complete orders
  
• Serve as a key communicator between providers, patients, and staff
  
• Document encounters and assessments using EMR systems
  

Qualifications:

• Certified and licensed as a PA in Minnesota
  
• Clinical experience with aging adults strongly preferred
  
• Effective communicator with great follow-through
  
• Valid driver’s license and reliable transportation
